胃癌高发区人群CagA+幽门螺杆菌与胃黏膜病变的关系

摘要
目的:探讨胃癌高发区CagA+Hpylori菌株与胃黏膜病变的相关性.

方法:对216例H pylori阳性的普通人群，行胃镜检查及胃黏膜HE染色明确胃黏膜病理学改变，同时用蛋白芯片技术检测H pylori的CagA抗体.
结果:在216例标本中，浅表性胃炎23例，萎缩性胃炎189例，中度以上萎缩性胃炎164例，占75.93%(164/216)，中度以上肠上皮化生(IM)31例，占14.81%(31/216)，中度以上异型增生33例，占15.28%(33/216)，胃癌4例.萎缩性胃炎、中度以上肠上皮化生和异型增生组CagA+Hpylori阳性率均较浅表性胃炎组显著升高(71.43%，67.74%，72.73% vs 17.39%，P<0.05).
结论:CagA+H pylori具有较强的促进胃黏膜萎缩、肠型化生和异型增生作用，感染CagA+Hpylori可能是胃癌发生的危险因素之一.
