【摘要】 目的 探讨血清红细胞生成素(Epo)水平在红细胞增多症患者中鉴别诊断的意义。方法 采用小鼠胎肝细胞培养红系集落的方法测定14例真性红细胞增多症(真红)患者及8例继发性红细胞增多症(继红)患者血清红细胞生成素水平，并同时测定红细胞计数、血红蛋白量、红细胞压积及继红患者氧分压、氧饱和度。结果 真红患者Epo水平为13.52±4.91U/L与正常人Epo48.0±17.7U/L比较有明显差异(P<0.01);继红患者Epo水平168.58±141.15U/L明显高于正常人。1例真红患者经放射性核素 32 P治疗血象好转后血清Epo水平自24.7U/L上升至190U/L。结论 真红患者血清Epo水平正常或降低，继红患者血清Epo水平增高，在鉴别诊断上有一定临床意义。真红治疗好转后，血清Epo水平上升;继红缺氧愈严重，血清Epo水平愈高(r=-0.925，P=0.001)。
