胃癌及癌前病变中PCNA、p53表达及其与Hpylori感染的关系

摘要
目的:探讨胃癌及癌前病变胃上皮细胞的增殖与幽门螺杆菌(Hpylori)感染的关系及H pylori感染致癌的可能机制.

方法:应用快速尿素酶试验与W-S银染色法将各种病变分为H pylori感染阳性组和阴性组，应用免疫组化SP法对各种病变胃上皮细胞增殖相关基因蛋白PCNA、p53的表达进行检测.
结果:PCNA、p53表达随病变进展逐渐增加，慢性浅表性胃炎组明显低于肠化生、不典型增生、胃癌组.且在浅表胃炎、肠化生组H pylori阳性者PCNA表达明显高于H pylori阴性者(68.7% vs 28.5%，93.8% vs 52%，P<0.05).
结论:H pylori感染可能通过促进PCNA、p53基因蛋白的过度表达而促进细胞增殖和恶性转化，且这种影响主要发生在胃黏膜癌变的早期阶段.
