重症急性胰腺炎并发急性肾功能衰竭44例

摘要目的:探讨重症急性胰腺炎(SAP)并发急性肾功能衰竭(ARF)的发病因素和防治措施.

方法:回顾分析我院2003年180例SAP病例中44例并发ARF和同期未发生肾功能衰竭的136例患者的临床资料.
结果:ARF多发生在起病10d内，90.27% 在3 d内发生;肾衰组48h体液隔绝量大于6 L的发生率、高脂血症发生率及血小板减少发生率高于非肾衰竭组(分别为40.9% vs 22.8%;40.9%vs 19.9%;65.9% vs 39.7% P = 0.03<0.05)，三酰甘油和乳酸脱氢酶的水平高于非肾衰竭组(7.1±6.2 mmol/L vs3.9±4.6 mmol/L;1 221.2±1 304.4 IU/L vs611.9±496.5 IU/L P<0.05);肾衰组CT评分、APACHEⅡ评分也高于非肾衰竭组(14.7±5.2 vs 9.8±3.8;7.5±2.5 vs 6.4±2.2 P =0.01<0.05);前者并发急性呼吸窘迫综合征(ARDS)、休克、脑病、感染者多于后者(分别为81.8% vs63.2%; 47.7% vs 25%; 36.4 vs 12.5% P = 0.02<0.05)，死亡率明显高于后者(45.5%vs 8.8% P = 0.000<0.05).

结论:SAP合并ARF是多因素作用的结果，与血流动力学紊乱、胰酶释放、炎症递质、细胞因子以及SAP伴发的其他脏器的功能损伤有密切关系.早期诊治SAP是预防其并发ARF的关键.
