摘要 目的： 探讨彩色多普勒血流影像(CDFI)评价肝动脉栓塞治疗肝癌的疗效及诊断其复发的价值。方法： 对36例肝癌患者栓塞前后CDFI、血管造影和CT做对照分析。结果： 36例中34例(94.4%)栓塞前CDFI显示有血流，分为4组图形：①肿瘤周边搏动性血流；②肿瘤内搏动性血流；③肿瘤周边连续性血流；④肿瘤内连续性血流。血管造影与CDFI对照，前2组发现相应血管，其余2组未见相应血管。栓塞2周后，36例中21例(58.3%)CDFI出现彩色血流；28例再次血管造影显示所有肿物供血动脉均有不同程度的再通。CDFI血流类型及血管造影显示的血管数与肿瘤大小有关。结论： CDFI评价肝癌栓塞疗效及诊断其复发的可靠性高，是一种无创性、可重复和经济的影像检查方法，可帮助确定再次治疗时间。
COLOR DOPPLER SONOGRAPHY INVESTIGATION OF TRANSCATHETER ARTERIAL EMBOLIZATION OF HEPATOCELLULAR CARCINOMA

Abstract Objective: To explore the value of color Doppler flow images(CDFI) in evaluation of the effectiveness of transcatheter arterial embolization(TAE) of hepatocellular carcinoma(HCC) and its recurrence.Methods: CDFI findings on 36 cases of HCC were evaluated and compared with those of angiography(AG) and CT before and after TAE.Results: Of 36 cases,34 cases(94.4%) were detected by CDFI before TAE.CDFI of the lesions fell into four patterns:①peritumoral pulsatile flow visualized in the periphery of the tumor;②intratumoral pulsatile flow shown in the central area of the tumor;③peritumoral continuous flow demonstrated in the periphery of the tumor;④intratumoral continuous flow found in the central area of the tumor.In comparison with AG,a feeding artery within or near the tumor capsule was found in the first two groups.No corresponding vessels were detected in the other two groups.Two weeks after TAE,CDFI was still identified in 21(58.3%) of the 36 cases and AG revealed recirculation of tumors' feeding artery in all the 28 cases who underwent AG once more.CDFI of the patterns and the number of feeding artery was corresponding to the tumor size.Conclusion: CDFI is quite reliable in evaluation of the effectiveness of TAE and tumor recurrence,which is a harmless,repeatable and inexpensive and helpful in determining the treatment time.
