摘 要：目的： 防止胰十二指肠切除术(Whipple手术)并发胰空肠吻合口瘘。方法： 在Whipple手术中，残胰胰管采用TH胶栓塞，胰腺残端缝合关闭，免除胰空肠吻合。结果： 共实行残胰管栓塞术42例，无1例术后胰瘘及手术死亡，术后恢复顺利，均痊愈出院；全部病例随访1～8年，无糖尿病发生。服用胰酶片能维持正常消化功能，生活质量良好。结论： 残胰胰管栓塞术，可有效防止Whipple术后并发胰瘘，降低手术死亡率，是一种较为理想的术式。

Duodenopancreactomy in complete obstruction of pancreatic duct

Abstract： Objective： To study the prevention of fistula formation in pancreaticojejunal anastomose after duodenopancreactomy(Whipple's operation).Methods ： In duodenopancreactomy,the stump of pancreatic duct was injected with TH glue.Embolism was formed and the pancreatic duct was closed.Consequently the pancreaticojejunostomy was avoided.Results ： This method was employed in 42 cases of Whipple's operations.Not a single case of postoperative pancreatic fistula or operative death occurred,the rate of successful operation has been raised.Conclusions ： The embolism formation of TH glue in pancreatic duct prevents effectively the complication of the pancreatic fistula in duodenopancreactomy and that reduces the operative mortality and time of hospitalization.
