【摘 要 】目的探讨内镜治疗对早期大肠癌的治疗价值。方法对1978年～1999年5月经病理确诊的91例早期大肠癌(98个癌灶)的临床资料、治疗方式和随访结果进行分析。结果接受单纯内镜治疗48例(51个癌灶)，外科手术治疗(包括内镜治疗后追加手术者)43例(47个癌灶)。内镜治疗的癌灶完全切除率达96.8%(61/63)，且无严重并发症产生。单纯内镜治疗组经3个月～12年随访，除失访4例、3例死于心脏病外，患者无1例复发，存活率达100%。比较45例接受单纯内镜治疗及25例手术治疗的粘膜层早期大肠癌的临床资料，结果内镜治疗组的疗效与手术治疗组相似(P>0.05)。结论早期大肠癌特别是粘膜层早期大肠癌的内镜治疗与手术治疗有同样的效果，且损伤小、安全、简便。

【 Abstract 】 Objective The result of endoscopic treatment for early colorectal cancer was evaluated. Methods Retrospective analysis of clinical and pathological documents of 91 cases with early colorectal cancer treated in past 21 years were carried out . Results Of the series 48(51 foci) received endoscopic management with extirpation in 96.8 % ; 43(47 foci) underwent surgery . The follow - up (for 3 months to 12 years) failed to show recurrent cancer except 4 lost in the course and 3 deaths from heart attack. Pathologically in early cancer with mucosa involvement only, no statistical difference (P >0.05) was found whether it was cured by endoscopy or surgery. Conclusion Endoscopic treatment of early colorectal cancer gives rise to similar efficacy and prognosis as traditional surgery in association with simplicity and safety.
