摘要 目的 观察老年人尿羟脯氨酸(HYP)的含量，评估其在老年男性骨质疏松症和糖尿病诊断中的意义。 方法 采用高效液相色谱法测定255例老年人空腹晨尿HYP含量，分为健康男性、女性、男性骨质疏松和2型糖尿病患者进行比较，并以38例青年人做对照。 结果 老年男性尿HYP的含量(194.8±75.4μmol/g Cr)与青年男性差异无显著性(P>0.05)；老年男性中，健康者、骨质疏松症患者及2型糖尿病患者之间差异均无显著性(均为P>0.05)；老年女性(315.4±97.0μmol/g Cr)显著高于老年男性及青年女性组(P<0.01)。 结论 尿HYP作为反映骨胶原快速转换的生化指标，对于老年男性骨质疏松症和2型糖尿病的诊断不具有特异性。

An investigation on urinary hydroxyproline content in the elderly

Abstract Objective To evaluate the significance of urinary hydroxyproline (HYP) in the diagnosis of osteoporosis and diabetes mellitus in aged. Methods Measurements of urinary HYP were made by using high pressure liquid chromatography in 38 healthy young adults aged 20-39yr as control and 255 old people aged 60-80yr including 38 heatlhy women, 165 heatlhy men, 35 patients with type 2 diabetes mellitus and 17 patients with osteoporosis. The results were expressed as the ratio HYP/creatinine (HYP/Cr). Results There was no significant difference between heatlhy old men (194.8±75.4μmol/g Cr) and young adults (P>0.05),the old patients with osteoporosis and with diabetes mllitus (P>0.05). But the mean value of HYP in old women (315.4±97.0μmol/g Cr) was higher than that of old men and young women (P<0.01). Conclusions The determination of urinary HYP is a biochemical marker of bone turnover in post-menopausal women, but is nonspecific for the old male patients with osteoporosis and diabetes mellitus.
