环丙沙星对胎儿软骨发育的影响

摘要 目的 了解孕妇应用喹诺酮类药物是否会影响胎儿的软骨发育。方法 采用微生物法测定了10例因感染而口服环丙沙星(ciprofloxacin, CPLX)的中晚期引产孕妇的血浆、羊水及胎儿血浆、软骨中药物浓度,并观察了胎儿软骨的形态学改变。结果 母血、羊水、脐血及胎儿软骨中CPLX浓度分别为1.83、1.35、0.65 mg/L及1.11 μg/g软骨。胎儿软骨结构在光镜下未见明显改变,但超微结构观察发现软骨细胞水肿、退形性变,基质丢失。结论 喹诺酮类药物能引起胎儿关节软骨细胞不同程度损伤,孕妇应尽量避免使用喹诺酮类药物。

Abstract Objective To study whether quinolone agents can penetrate the placenta and what is its level in fetal cartilage. Methods The concentrations of ciprofloxacin in maternal blood plasma, amniotic fluid and fetal blood plasma and cartilage were determined in ten pregnant women taking oral ciprofloxacin because of infection and undergoing termination of gestation between 19 and 34 weeks. At the same time, the effect of the concentrations of ciprofloxacin on the morphology of fetal cartilage was tested. Results The concentrations of ciprofloxacin in maternal blood plasma, amniotic fluid, fetal blood plasma and cartilage were 1.83, 1.35, 0.65 mg/L and 1.11 μ g/g cartilage respectively. Although there was no significant change of the fetal cartilage in light microscopy, yet specific alteration was found in electron microscopic studies. Conclusion Ciprofloxacin can cause lesions in fetal cartilage and its use should be avoided in pregnant women.
