【摘要】 目的 探索修复手指中末节皮肤脱套伤的新术式。方法 对10例10指中末节皮肤脱套伤，选用带有少量末节趾骨的第二趾甲皮瓣进行修复。皮瓣切取长度2.2 cm ～ 6.1 cm，平均2.9 cm。皮瓣游离平面位于趾伸屈肌腱表面。 皮瓣血供重建方式为趾 -指血管吻合。 结果 术后皮瓣全部成活，随访5 ～ 10个月，伤指修复后外形及屈伸功能均满意。两点辨别觉为6 ～ 9 mm。 结论 用第二趾甲皮瓣一期修复手指中末节皮肤脱套伤，伤指有指甲和丰满的指腹，可最大限度地恢复伤指的外形和功能。

Transplantationof nail skin flap from the second toe for repair

    of degloving injury of the fingers

【Abstract】 Objective Tointroduce a new operating procedure for repair of degloving injuries of the fingers. Methods Ten degloved fingers in ten cases were repaired by nail skin flapfrom the second toe with a part of the distal phalanx. The length of the flaps ranged from2.2 cm to 6.1 cm, being 2.9 cm on average. The dissection level was on the surface of theflexor and extensor tendons. The blood supply of the flaps was restored by anastomosingthe arteries, veins and nerves of the flaps with the vessels and nerves of the fingers.After the operation, the fingers were immobilized and the anticoagulants, antispasmic andantibiotic drugs were used. Results All the flaps survived. Satisfactory appearance and good functionwere obtained in ten fingers with normal sensation and 2 - PD of 6 ～ 9 mm after 5 ～ 10 months'follow - up. Conclusions Transplantationof the nail skin flap from the second toe could primarily repair the defects of skin, nailand finger pulp of the degloved fingers. The function and the appearance of thereconstructed fingers was satisfactory.
