颅内海绵状血管瘤的影像学诊断

【摘要】 目的 分析颅内海绵状血管瘤的MRI、CT和DSA的表现。方法 经手术病理证实的颅内海绵状血管瘤76例,其中脑内型海绵状血管瘤59例,中颅窝鞍旁硬膜型海绵状血管瘤17例。28例行CT扫描,73例进行MRI检查,20例行DSA检查。结果 脑内型海绵状血管瘤MRI表现为低信号环围绕的桑葚状混杂信号团块,DSA检查未见异常染色。中颅窝、鞍旁的病灶CT表现为较均匀的稍高密度影,MRI呈类似哑铃形较均匀的稍长T1 信号影,T2 WI 和质子密度像为高信号影,增强扫描多见明显均匀强化。DSA多表现为毛细血管期延至窦期的淡染色影。结论 颅内海绵状血管瘤的MRI表现比较有特异性,综合MRI、CT和DSA所见有利于正确诊断。

Imaging diagnosis of intracranial cavernous hemangioma

【Abstract】 Objective To analyze CT,MRI, and DSA findings of intracranial cavernous hemangioma. Methods 76 cases of intracranial cavernous hemangioma with pathologic confirmation were analyzed. 59 cases were intra-axial lesions, 17 cases were dural cavernous hemangiomas in middlefossa. 28 cases had CT scan, 20 cases had DSA examination. Results Intra-axial cavernous hemangioma often showed slight high density mass on CT images and mulberry-like mixed signals with hypointensity ring on MRI. 17 cases of parasellar lesions had dumb-bell shape, homogeneous slight hyperintensity on CT images, and homogenous slight hypointense on T1 WI and hyperintense on T2 WI and proton density images. After injecting contrast medium, the mass markedly enhanced. Faint staining from capillary stage to sinus stage could be seen on DSA images. Conclusions The MRI findings of intracranial cavernous hemangioma are specific, combined examination could help to establish a correct diagnosis. MRI is superior to CT in detecting intracranial cavernous hemangioma.
