【摘要 】 目的 探讨胃肠道恶性肿瘤术后肿瘤复发性肠梗阻的临床特点以及病因诊断的要点。方法 以手术探查结果作为标准，回顾性地对91例胃肠道恶性肿瘤术后肠梗阻病人在病理、病程、临床症状和体征、癌胚抗原(CEA)检测等方面进行统计分析。结果 肿瘤复发组病人原发肿瘤的分化程度较非复发组低下，肿瘤分期也较晚(P<0.05)，复发组肠梗阻病程明显长于非复发组(P<0.01)；两组在肠梗阻主要症状、腹部包块、不完全性肠梗阻和结、直肠梗阻的发生率上均存在显著差异(P<0.01)；复发组CEA阳性率和平均值(13.06±10.36)μg/L，均明显高于非复发组(P<0.05)。结论 两组间的上述差异点可作为胃肠道恶性肿瘤术后肿瘤复发性肠梗阻病因诊断的要点因素。

Clinical characteristics of intestinal obstruction due to recurrent gastrointestinal carcinoma

【Abstract 】 Objective To investigate the main etiological factors and clinical characteristics of intestinal obstruction after operation for gastrointestinal cancer. Methods Compared with surgical outcomes, many indexes of 91 patients with intestinal obstruction after operation for gastrointestinal carcinoma were analysed retrospectively, including pathology, disease course,clinical manifestations and serum level of carcinoembryonic antigen(CEA).Results Compared with benign intestinal obstruction group,primary carcinoma of recurrent group had lower degree of differentiation, later staging(P<0.05), and longer disease course (P<0.01). Significant difference in incidence were observed between recurrent and benign group in main symptoms of intestinal obstruction, abdominal mass, uncomplete obstruction and large bowel obstruction (P<0.01). The mean level of CEA was (13.06±10.36)μg/L in recurrent group and (5.23±4.71)μg/L in benign group (P<0.05). Positive rate of serum CEA in recurrent group was higher than that in benign group (P<0.05). Conclusion All the factors mentioned above could be used in etiological diagnosis of intestinal obstruction after operation for gastrointestinal carcinoma.
