颈部颈丛和臂丛神经鞘瘤的彩色多普勒超声定位诊断

摘 要 目的:提高超声对颈丛和臂丛神经鞘瘤诊断的准确性。方法:应用10MHz高频彩色多普勒超声对经临床、手术及病理证实的40例颈部椎旁间隙神经鞘瘤(颈丛12例,臂丛28例)与周围组织关系进行探查,从而准确解剖定位。结果:颈丛和臂丛神经鞘瘤均位于椎旁间隙。颈丛神经鞘瘤位于颈动脉分叉水平及其上方,头长肌或前斜角肌后外方;而臂丛神经鞘瘤位于颈动脉分叉水平以下,紧贴前斜角肌后或后外方,颈横动脉横跨前斜角肌表面向肿瘤未被肌肉覆盖部分的浅面走行。结论:通过彩色多普勒超声对颈丛和臂丛神经鞘瘤的准确解剖定位,可提高其对肿瘤诊断的准确性。

Color Doppler UltrasoundLocational Diagnosis for Neurilemmoma of the Cervical Plexus and Brachial Plexus

ABSTRACT Objective:To raise theaccuracy of ultrasonic diagnosis for neurilemmoma of the cervical plexus and the brachialplexus.Methods:10 MHz high frequency color Doppler ultrasound was used to detect theparavertebral neurilemmoma of 40 patients,12 cases of which were from the cervicalplexus,and others from the brachial plexus,All the cases were confirmed by clinical、surgicaland pathological findings.The relationships between the tumors and their surroundingtissues were investigated,so as to get their accurate anatomic locations.Results:Both theneurilemmomas of the cervical and the brachial plexuses are in the paravertebralspace.Neurilemmoma of the cervical plexus lies on or obove the carotidbifurcation,posterolateral to the longus capitis muscle or the anterior scalenemuscle;while neurilemmoma of the brachial plexus lies below the carotid bifurcation,andposterior or posterolateral to the anterior scalene muscle.The transverse cervical arterywhich crosses the anterior scalene muscle covers the part of the tumor and liessuperficially.Conclusions:Color Doppler ultrasound may raise the diagnostic accuracy forneurilemmoma of the cervical and the brachial plexuses by investigating their anatomiclocations.
