两种剂量生长抑素序贯治疗静脉曲张破裂反复大出血

目的 : 观察两种剂量生长抑素(施他宁)序贯应用治疗肝硬化食管胃底静脉曲张破裂反复大出血的治疗效果和安全性.
方法 : 反复大出血患者47例，分为2组，施他宁250 mg/h和500 mg/h序贯剂量组26例，施他宁250 mg/h常规剂量组21例.
结果 : 再次止血开始时间序贯组为3.22±1.39 h，常规剂量组为9.25±6.04 h，有显著差异性(P<0.001);再次出血停止时间序贯组为5.11±1.62 h，常规剂量组为14.87±9.11 h，有显著差异性(P<0.01);24 h和48 h止血成功率序贯组88.46%(23/26)和96.15%(25/26)，明显高于常规剂量组的47.61%(10/21)和71.43%(14/21)(P<0.005和P<0.01).近期再出血率:序贯组7.69%(2/26)，常规剂量组28.57%(6/21)，有显著差异性(P<0.005).平均输血量:序贯组384.62±137.67 mL，少于常规剂量组的580.95±153.68 mL，有显著差异性(P<0.05).死亡率:序贯组3.85%(1/26)，常规剂量组33.34%(7/21)，有显著差异性(P<0.05).两组均未发生明显不良反应.

结论 : 两种剂量施他宁序贯治疗对重度肝硬化食管静脉曲张破裂反复大出血的疗效优于常规剂量组，且止血时间快，近期再出血率低，死亡率下降，无不良反应.
