【摘要】目的回顾性分析放射治疗扁桃体癌的疗效及预后因素。方法107例经病理证实的扁桃体癌均行单纯根治性放射治疗。根据1997年国际抗癌联盟(AJCC)分期标准，Ⅰ期5例，Ⅱ期14例，Ⅲ期27例，Ⅳ期61例。生存分析采用Kaplan-Meier法及Logrank检验，多因素分析采用Logistlc回归模型。结果全组5年总生存率为45.8%。Ⅰ～Ⅳ期5年生存率分别为89.6%、72.2%、54.3%和38.9%。多因素分析显示AJCC分期为主要的预后因素。结论AJCC分期为主要的预后因素，单纯放射治疗对Ⅰ、Ⅱ期扁桃体癌病变可取得较好的疗效。

    【Abstract】ObjectiveTo evaluate the therapeutic outcome and define the prognostic factors in patients with tonsillar carcinoma after radical radiotherapy.MethodsFrom Jan.1978 to Dec.1998,107 patients with tonsillar carcinoma proved pathology,treated primarily with definetive radiotherapy were eligible for the present analysis.According to TNM staging system,there were 5 Stage Ⅰ,14 Stage Ⅱ,27 Stage Ⅲ and 81 Stage Ⅳ patients.The significance of prognostic variables in the survival expectancy was evaluated with multivariate analysis and Cox stepwise regression model.ResultsThe 5-year total survival rates were 49.4%.The 5-year survival rates based on stage were 89.6%of Stage Ⅰ,72.2% of Stage Ⅱ,54.3% of Stage Ⅲand 38.9% of Stage Ⅳ,respectively(P=0.003).Multivariate analysis showed that AJCC stages were the significant prognostic factors(P<0.05).ConclusionRadiotherapy for tonsillar carcinoma is a primary treatrment for the selection,especially for atage I and II patients who are likely to give good results. ......
