【摘要】 目的 改良手术方法并加强合理的围手术期治疗，以减少Chid-Pugh C级肝硬化患者的手术死亡率。方法 回顾性分析Child-Pugh C级肝硬化行脾切除术49例。取正中切口，分别用传统方法行脾切除术19例，原位法手术组30例，围手术期治疗因病施治，重点突出。结果 49例中5例死亡，传统方法组死亡5例，手术死亡率为26.3%，原位法手术组死亡率为0。结论 在手术操作熟练、方法得当的情况下，Child-Pugh C级肝硬化患者可以行脾切除手术治疗。【关键词】 Child-Pugh C级；手术死亡率；原位法脾切除术；围手术期治疗

【Abstract】 Objective To reduce the operative mortality of patients with cirrhosis of Child-Pugh C by improving operative procedures and strengthening perioperative treatment.Methods The clinical data of 49 patients with cirrhosis of Child-Pugh C receiving splenotomy were retrospectively.analyzed Conventional procedures of splenotomy were used for 19 patients and in situ procedures were used for 30 patients by median incision,taking treatment suited to conditions and stressing the main points in perioperative period.Results 5 patients were died in the 49 patients,all 5 the patients were part of the group of conventional procedures and there was no death in situ procedures.Operative mortality was 26.3%.Conclusion Under the conditions of perfect operative procedure and proper operative method,splenotomy is feasible for patients with cirrhosis of Child-Pugh C. ......
