宫颈上皮内瘤变106例临床分析

摘要： 目的： 探讨宫颈上皮内瘤变(CIN)的诊治策略和全子宫切除术(切除阴道上段1 cm)在CINⅡ级、CINⅢ级治疗中的地位。 方法： 对106例行全子宫切除术的CINⅡ级、CINⅢ级病例资料进行回顾性分析，总结其发病年龄、临床症状特点，以及行宫颈液基薄层细胞学涂片(经TBS系统报告)、阴道镜下多点活检术两种检查方法对CINⅡ级、CINⅢ级的诊断价值。结果：CINⅡ级、CINⅢ级在35～45岁年龄段患病人数最多(60例,56.6%)。临床多无特殊症状，其中无症状体检发现41例(38.7%)，因不规则阴道流血检查发现37例(包括接触性出血，34.9%)。术前宫颈液基薄层细胞学涂片与病理符合率为64.7%，术前经阴道镜下活检与术后诊断符合率为90.2%，术后病理诊断升级为9.8%，术前诊断为CIN术后发现为浸润癌7例(6.6%)。结论：阴道镜下多点活检是CIN诊断的一个重要方法。全子宫切除术对于无随访条件及术前不能排除浸润癌的CINⅡ级、CINⅢ级患者的治疗是有必要的。关键词： 宫颈上皮内瘤样病变； TBS诊断系统； 阴道镜； 活体组织检查
Cinical analysis of one hundred six patients with cervical intraepithelial neoplasia grade Ⅱand grade Ⅲ

Abstract: Objiective: To investigate the methods of diagnosis and treatment of Cervical Intraepithelial Neoplasia (CIN), the position of complete hysterectomy (to cut vagina 1 cm)in the treatment of Cervical Intraepithelial Neoplasia grade Ⅱand grade Ⅲ. Methods: To investigate the age of onset, the symptomatic feature and the diagnostic value of cervical cytological smear, colposcopically multiple biopsy in the treatment of 106 patients with CIN Ⅱ、 CINⅢ were analyzed retrospectively. Result: The stage of age that the number of patient was most is 35～45. Many of them were no symptom, 41 patients(38.7%) dislayed no any symptom, 37 patients(34.9%) displayed irregular colporrhagia. The coincidence between cervical cytological smear and colposcopically multiple biopsy is 64.7%. The coincidence between preoperative colposcopically biopsy and postoperative pathological diagnosis is 90.2%.7 patients diagnosed CINⅢ before operation were infiltrating carcinoma after operation. Conclusion: The stage of age that the number of patient was most is 35～45. Many of them are no symptom. Colposcopically multiple biopsy is a significant method to diagnose CIN. Complete hysterectomy (to cut vagina 1 cm) is necessary for the patients with no followup condition and no remove infiltrating carcinoma before operation. ......
