肺癌放疗所致放射性肺损伤的肺灌注改变

【摘要】 目的 探讨99mTc-MAA肺灌注显像和常规肺功能检查在肺癌放疗所致放射性肺损伤中的临床应用价值。方法 52例肺癌患者均接受常规根治性放疗及在放疗前、中、后均进行肺灌注显像和常规肺功能检查;治疗结束后定量分析放射性肺损伤的肺灌注功能变化及其严重程度。结果 肺癌放疗后放射性肺损伤的发生率约为38.4%;肺癌患者放疗前、后用力肺活量(FVC)、第一秒用力呼气量(FEV1)、弥散系数(DICO)的变化值比较差异有显著性(P均<0.05);肺癌患者放疗前、后患侧肺灌注功能损害的定量变化对比和严重程度对比差异均有显著性(P<0.05)。结论 肺灌注显像可以定量监测肺癌患者放疗前、后患侧肺灌注功能的变化,并可以预测放射性肺损伤的发生及其严重程度。【关键词】 肺癌;辐射损伤;放射治疗;放射性核素显像
Influence of pulmonary perfusion function for radiation pulmonary injury caused by radiotherapy for lung carcinoma

【Abstract】 Objective To explore the clinical application value in radiation pulmonary injury caused by radiotherapy for lung carcinoma with 99mTc-MAA pulmonary perfusion imaging and the pulmonary function examination.Methods 52 patients with lung cancer were treated by radical radiotherapy and quantitatively analyzed injured pulmonary perfusion function before and after radiotherapy examined by pulmonary perfusion imaging and the pulmonary function.Results The incidence rate of radiation pulmonary injury after radiotherapy for lung carcinoma was 38.4%.The difference was significant between the pulmonary function FVC,FEV1,DICO before and after radiotherapy for lung carcinoma patients,P<0.05.Quantitative influence and the severe degree of injured pulmonary perfusion function before and after radiotherapy for lung cancer patients were obviously different,P<0.05.Conclusion It can predict the occurrence and the severe degree of radiation pulmonary injury to estimate quantitatively pulmonary perfusion function influence pre-post-radiotherapy with pulmonary perfusion imaging. ......
