颅脑损伤合并腹部闭合伤76例诊疗体会

【摘要】 目的 探讨颅脑损伤合并腹部闭合性损伤病人的诊断思路及治疗原则。方法 回顾性研究我科1992～2005年收治的颅脑损伤合并腹部闭合性损伤病人76例的临床资料。结果 及早明确诊断和抢救休克可明显降低病人的死亡率。为了减少腹部闭合性损伤的漏诊率，应行反复、多点腹腔穿刺，尽可能利用B超和CT在腹内脏器伤诊断中的重要作用。结论 在治疗上既不能因强调开颅血肿清除的重要性而延误剖腹探查的时机，也不能因强调剖腹探查而忽视开颅血肿清除的重要性，应把抢救生命放在首位。【关键词】 颅脑损伤；腹部损伤；外科；手术

【Abstract】 Objective To discuss the diagnosis way of thinking and the treatment principle of craniocerebral injury to merge the closing ventral injury. Methods Analysed 76 patients with craniocerebral injury to merge the closing ventral injury in my branch between 1992 and 2005.Results Early diagnosis and rescuing shock can lower the death rate obviously. For reducing miss examining rate of the patients of closing ventral injury, should go again and again and order abdomen more to wear.Make use of B ultrasonic and CT possibly in the internal organs the diagnosis of important function.Conclusion On the treatment since cant emphasize importance of operating skull to clear hematoma but delay the opportune moment of exploration operating abdomen, nor to emphasize importance of exploration operating abdomen but neglects to operate skull to clear hematoma should put the salvage life at first. ......
