纤维支气管镜下介入微波治疗大气道狭窄的疗效评估

摘要:目的以肺功能改善情况为指标,评价使用微波治疗由良性瘢痕狭窄和恶性肿瘤增生所导致的气道阻塞患者2月内的近期疗效。方法选择15名临床确诊的大气道(叶支气管)阻塞病人,其中7名由支气管慢性炎症引起良性瘢痕狭窄所致,8名因中心型肺癌腔内型所致,对病人统一进行纤维支气管镜下介入微波治疗3次。以肺活量、第1秒用力肺活量、最大呼气中段流量、最大通气量为观察项目,于术前及术后2、15、30、60 d分别进行常规肺通气功能检测。结果 术后2 d肺通气功能开始有所改变,良性瘢痕狭窄者术后15、30、60 d肺通气功能基本维持稳定。恶性肿瘤导致的阻塞性改变,治疗后15、30 d肺通气功能也呈有意义改善,但60 d后肺通气功能明显下降。结论 微波治疗气道良性狭窄效果明显,且可维持肺通气功能在良好水平达2月以上;恶性肿瘤导致的气道阻塞亦有较好的近期临床效果,但2月后肺通气功能下降显著,应酌情进一步处理。关键词:支气管镜;微波治疗;气道狭窄

Abstract:ObjectiveTo understand the effect of microwave treatment due to obstruction by scar and lung cancer.Method Sampling 7 patients of scar and 8 patients of lung cancer, intervene microwave treatment 3 times. Lungventilate faction was tested before operation and 2days, 15days, 30days, 60days after operation (vital capacity, forced expiratory volume in one second, maximal midexpiratory flow and maximal voluntary ventilation) . Result2days after operation, lungventilate faction began change better. The patients of obstruction by scar′lungventilate faction was normal after 15days, 30days and 60days of operation. The lung cancer patients of lungventilate faction was good after 15days and 30days too. But it came down obviously after 60days. ConclusionThe effect of benign tracheanarrow by microwave treatment was well. It can maintain about 2 month long. The early effect of vicious tracheanarrow was well, but lungventilate faction came down obviously after 2 month. It must be handled. ......
