【摘要】 目的 观察胃癌临床综合性治疗的疗效。方法 回顾分析1998年11月～2004年12月我科收治229例胃癌的临床治疗资料，观察手术并发症及化疗副作用及术后3、5年生存率。结果 治疗组无手术死亡，临床主要并发症为：不同程度的肠粘连为17.9%(41/229)，反流性食管炎发生率为33.62%(77/229)。而无其他严重术后并发症，3年、5年生存率分别为45.22%、29.62%。7例Ⅳ期胃癌患者(合并肝转移)经腹腔化疗后，CR：28.57%，PR：14.29%RR：42.86%。结论 胃癌的治疗应以手术治疗为主的综合性治疗为原则，争取早发现，早诊断，早治疗；合理选择手术方式，进行规范性手术，合理选用各种辅助治疗；对进展期近端胃癌应首选根治性全胃切除术；对进展期胃癌力争行腹腔化疗；对晚期患者亦采取积极治疗的态度，达到延长患者的生存期。
