【摘要】 目的 探讨吻合器痔切除术(procedure for prolapse and hemorrhoids,PPH)治疗老年重度混合痔的应用价值。方法 对我院普外科收治的114例Ⅲ～Ⅳ期以内痔为主的高龄环状混合痔患者应用PPH和Milligan法进行治疗，并对两种术式疗效、手术时间、住院时间、恢复工作时间、并发症等进行全面比较分析。结果 两组病人手术过程顺利，然而与Milligan手术方式相比，PPH手术方式可明显缩短手术时间和住院时间、减少术后肛周疼痛(P<0.01)；同时在术后肛门狭窄率、痔核脱出率及感觉性大便失禁率等方面，Milligan组发生率分别为13.2%、15.1%和32.1%，而PPH组分别为0、3.3%和18.0%，PPH组疗效优于Milligan组(P<0.05)。结论 PPH治疗老年重度混合痔安全有效。

    【Abstract】 Objective To investigate the clinical effect of PPH in the treatment of gerontism severe mixed hemorrhoids.Methods One hundred and fourteen patients with severe mixed hemorrhoids,hospitalized to our hospital from 2002 to 2005,were treated with PPH and Milligan-Margan.Operative time,length of hospitalization,time return to work and complication of two groups were compared.Results The process of operation in two groups was successful.Howerver,operative time,length of hospitalization,pain of post-operation of PPH group were reduced obviously compared with that of Milligan group(P<0.01)；the rate of archostenosis,hemorrhoid exodus,copracrasia of PPH group was decreased compared with that of Milligan group(P<0.05).Conclusion PPH is an effective and safe method for the treatment of gerontism severe mixed hemorrhoids. ......
