丙泊酚TCI麻醉在乳腺癌根治术中的应用

【摘要】 目的 研究乳腺癌手术丙泊酚靶控输注麻醉达到不同麻醉水平的效应室浓度参考范围。方法 60例择期行乳腺癌根治术患者，ASA Ⅰ～Ⅱ级，应用国产TCI-Ⅰ型泵实施丙泊酚靶控输注，间断辅以芬太尼镇痛，记录意识消失时间、意识消失时效应室浓度、麻醉维持阶段效应室浓度、苏醒时间及其效应室浓度。结果 患者意识消失、麻醉维持期以及苏醒时效应室浓度分别为(3.38±0.62)μg/ml、(5.48±0.51)μg/ml、(2.10±0.42)μg/ml，意识消失时间和苏醒时间分别为(98.26±14.62)s和(13.55±4.52)min，且意识消失浓度与苏醒浓度之间差异有非常显著性(P<0.01)，表明苏醒浓度明显低于意识消失浓度。结论 国产TCI-Ⅰ型泵输注丙泊酚应用于乳腺癌根治术麻醉获得良好的效果，并明确了其各输注参数，对临床应用TCI技术具有一定的指导作用。【关键词】 丙泊酚；靶控输注，乳腺癌

【Abstract】 Objective To determine the diprivan concentration of effect locus in TCI anesthesia with Diprivan PFS system to operation of breast cancer.Methods 60 patients receiving operation of breast cancer,ASA Ⅰ-Ⅱ,diprivan TCI anesthesia with fentanyl interval.Record time and diprivan concentration of consciousness disappear and palinesthesia,diprivan concentration in anesthesia maintaining.Results Diprivan concentration of consciousness disappear,anesthesia maintaining and palinesthesia is (3.38±0.62)μg/ml,(5.48±0.52)μg/ml,(2.10±0.42)μg/ml.Time of consciousness disappear and palinesthesia is (98.26±14.62)s and (13.55±4.52)s.There are significant discrepancy in diprivan concentration of consciousness disappear and palinesthesia (P<0.01).Conclusion The effect in TCI anesthesia with diprivan to operation of breast cancer is accuracy and can direct clinical application of TCI. ......
