原发性高血压及左室肥厚患者血清 IGF1和胰岛素抵抗的研究

     摘要: 目的:研究胰岛素样生长因子1(IGF1)和胰岛素抵抗(IR)与原发性高血压及左室肥厚的关联性。方法:78例分为3组,原发性高血压(EH)组29例, EH合并左室肥厚(LVH)组28例,正常对照组21例。采用二维超声心动图测定3组左心室重量指数(LVMI),采用放射免疫法测定3组血清IGF1及胰岛素(INS)水平,并计算胰岛素敏感指数(ISI)。结果: EH+LVH组血清IGF1和INS 高于EH组,EH组高于正常对照组,差异均有统计学意义(P<0.01);相关分析结果显示: IGF1与INS呈正相关,与ISI呈负相关。结论:EH患者血清IGF1和INS水平升高,合并LVH时升高更显著;IR促进LVH的作用可能与其升高IGF1 的水平有关。

    The relationship between insulinlike growth factor1 and insulin resistanceQIU Hongmin, ZHOU Xiaohui, MU Yuming, et al

    Abstract: Objective: To study the ralationship between insulinlike growth factor1 (IGF1), insulin resistance (IR) and left ventricular hypertrophy (LVH) in essential hypertension (EH). Methods: Twentynine essential hypertension patients, twentyeight essential hypertension patients with left ventricular hypertrophy and twentyone norotensive subjects were studied using echocardiography to determined left ventricular mass index (LVMI) and left ventricular diastolic function. Serum insulinlike growth factor1, insulin levels were measured by specific radioimmunoassay and the insulin sensitive index was calculated. To apply the analysis of variance, rank sum test and linear correlation to analyse the data. Results: Essential hypertension patients had a higher level of serum insulinlike growth factor1, insulin and had a lower insulin sensitive index than control group (85.96±35.52 ng/ml vs 130.24±28.79 ng/ml, 8.92±6.46 μU/ml vs 18.08±12.05 μU/ml, -3.42±0.99 vs -4.39±0.96, P<0.01) ; Compared with essential hypertension patients, the levels of serum insulinlike growth factor1 and insulin were higher, in the insulin sensitive index was lower essential hypertension patients with left ventricular hypertrophy (130.24±28.79 ng/ml vs 215.79±71.73 ng/ml, 18.08±12.05 μU/ml vs 25.09±16.06 μU/ml, -3.49±0.96 vs -4.80±1.04, P<0.01). There was a significantly correlation between the serum insulinlike growth factor1 and insulin, insulin sensitive index (r=0.352 2, r=-0.363 5, P<0.05). Conclusions: Study shows that serum insulinlike growth factor1 levels increased in patients with essential hypertension, especially in those with left ventrciular hypertrophy ; The essential hypertension patients and the essential hypertension patients with left ventricular hypertrophy have high levels of insulin and insulin resistance; In patients with left ventricular hypertrophy, The insulin resistance may play a role indirectly by enhance the serum insulinlike growth factor1 levels. ......
