【摘要】 目的 探讨重建钢板配合中空螺钉对肱骨髁上髁间骨折的治疗效果及影响因素。方法 回顾性分析我科自1999年2月~2004年4月用此方法治疗的此类骨折资料完整病例28例，男22例，女6例，年龄15~69岁，平均43.8岁。开放性骨折7例，闭合性骨折21例；左侧12例，右侧16例；合并尺神经损伤5例。按AO肱骨髁间骨折分型：C1型9例，C2型12例，C3型7例。术中尺神经前移22例。手术入路：经肱三头肌舌行瓣入路17例，经尺骨鹰嘴截骨入路5例，经肱三头肌双侧入路4例，合并鹰嘴骨折经骨折端入路2例。结果 28例患者均经6~36个月，平均14.3个月的随访。骨折均已达到骨性愈合。采用Cassebaum评分系统进行疗效评价，优16例，良7例，可3例，差2例。优良率82.1%。发生尺神经损伤2例，经治疗后恢复。肘关节功能受限2例。结论 应用重建钢板和中空螺钉可以对肱骨髁间骨折进行早期的解剖复位和牢固固定。配合早期适当的功能锻炼，肘关节的功能良好。
