血清C反应蛋白在急性脑梗死患者临床评估中的价值

【摘要】目的 探讨急性脑梗死(ACI)患者血清C反应蛋白(CRP)水平的变化，评估CRP在ACI患者神经功能缺损程度及近期预后中的临床价值。方法 采用免疫透射比浊法检测68例ACI患者(ACI组)、21例腔隙性脑梗死患者(LCI组)和40名健康体检者(对照组)血清CRP水平，计算其异常率并进行比较。对病例组患者在入院时及住院15 d时行临床神经功能缺损程度评分(NDS)，计算出评分差值并进行比较。结果 ACI组、LCI组CRP水平均高于对照组(均P<0.01)，且ACI组CRP水平高于LCI组(P<0.05)；89例患者中CRP异常率50.56%，且ACI组CRP异常率高于LCI组(P<0.05)；入院时NDS≥15分组CRP水平明显高于NDS<15分组(P<0.01)，NDS评分和CRP水平呈正相关(r=14.31，P<0.01)；住院15 d时NDS评分差值少组比NDS差值多组CRP含量高，差异有显著意义(P<0.05或P<0.01)。结论 CRP水平是临床评估ACI患者神经功能缺损程度和近期预后的重要生物学指标。【关键词】 C反应蛋白；脑梗死，急性；神经功能缺损程度评分；预后
The value of serum Creactive protein in clinical evaluation of patients with acute cerebral infarction

【Abstract】 Objective To investigate the serum level of Creactive protein (CRP) in patients with acute cerebral infarction (ACI),and to evaluate the clinical value of CRP in the degree of nervous function defect and recent prognosis in ACI.Methods 68 patients with ACI (ACI group) and 21 patients with lacunar infarction (LCI group) as well as 40 healthy subjects as control group were enrolled in this study. The serum levels and abnormal rate of CRP were determined.The concentration of CRP was measured by immune transmission turbidity method.All the patients were scored by clinical neurological function deficit scale (NDS) at the first day and 15th day after admission respectively,then compared among 3 groups.Results The serum levels of CRP in ACI group and LCI group were higher than those in control group (P<0.05).It was also higher in ACI group than that in LCI group (P<0.05).The higher abnormal rate of CRP was found in ACI group than in LCI group(P<0.05).The serum levels of CRP in patients with NDS scores≥15 was significantly higher than those with NDS scores<15 (P<0.01),NDS scores were 1inear1y positively correlated with CRP (r=14.31,P<0.01). Higher CRP levels were found in patients with lower difference of NDS scores at the 15th day after admission (P<0.05 or P<0.01).Conclusion Serum level of CRP is an important biological marker to evaluate the degree of nervous function defect and recent prognosis in ACI. ......
