     [摘要] 目的：探讨黄色肉芽肿性肾盂肾炎CT诊断价值。方法：回顾分析5例经手术病理或/和穿刺活检证实黄色肉芽肿性肾盂肾炎CT表现，5例均行CT平扫加增强检查，4例静脉肾盂造影，3例超声检查。结果：5例均为单侧发病，右肾1例，左肾4例。平扫4例肾体积普遍增大，1例肾内见多个囊状低密度灶，3例见多个结节状低密度灶；1例肾脏局部肿大，肾实质见局灶性低密度灶。增强5例低密度灶无强化，病灶边缘强化。3例合并肾结石，1例合并输尿管上段结石，5例有肾周炎症。结论：CT检查能显示病灶部位、范围及肾周受累程度，对黄色肉芽肿性肾盂肾炎有重要的诊断价值。

    Role of Computerized Tomography in Diagnosing Xanthogranulomatous Pyelonephritis

    Abstract：Objective To probe role of computerized tomography (CT) in diagnosing xanthogranulomatous pyelonephritis.Methods 5 cases of xanthogranulomatous pyelonephritis,which were confirmed by surgical pathology and/or puncture biopsy,underwent plain and enhanced scan with CT,and their imaging data were reviewed.In addition,4 cases underwent intravenous pyelography and 3 cases underwent ultrasound examination.Results All cases had lesions in only one side,with one situated in right kidney and 4 in left kidney.4 cases were found universal enlargement of kidney under plain scan,and one case was found multiple cystlike low intensity lesions.3 cases were found multiple nodulelike lowintensity lesions.One case had localized enlargement of kidney,displaying localized lowintensity lesion in renal parenchyma.As for contrast enhanced scan of the 5 cases,none of the lowintensity lesions were enhanced,except for the edge being enhanced.3 cases were complicated with paranephritis.Conclusion CT is able to detect location,spread and perirenal involvement of xanthogranulomatous pyelonephritis,playing an important role in diagnosing xanthogranulomatous pyelonephritis. ......
