窒息新生儿血气及阴离子间隙状态分析

【摘要】 目的 了解窒息新生儿窒息后酸碱失衡及阴离子间隙状况。方法 对22例窒息新生儿同步做血气分析及电解质测定,计算阴离子间隙(AG)值。结果 高AG 16例,正常AG 5例,低AG 1例,代谢性酸中毒20例,发生率为90.9%,高AG代谢性酸中毒占代谢性酸中毒的80%,三重酸碱失衡占20%。结论 分析结果表明,新生儿窒息后存在明显甚至是严重的酸碱失衡,而且以高AG状态为主,代谢性酸中毒发生率高。高AG代谢性酸中毒与窒息的程度无关(P>0.05);高AG组血Cl-、HCO3-比正常AG组、低AG组明显降低(P<0.05)。故窒息新生儿除做血气分析外,需做AG分析,以便对酸碱紊乱做出更准确的诊断,以指导纠正酸碱失衡及电解质紊乱,同时必须积极疏通气道,改善微循环。【关键词】 新生儿;窒息;阴离子间隙
The analysis of blood gas and anion gap in newborns with asphyxia

【Abstract】 Objective To investigate the situation of acid base imbalance and AG in newborn with asphyxia.Methods 22 cases of neonatal asphyxia were selected.Blood gas analysis was done.The serum electrolytes was measured simultaneously,then calculated the values of AG.Results There were 16 cases of high AG level,5 cases of normal level,1 case of low AG level,20 cases of metabolic acidosis and its incidence rate is 90.9%.Among the cases of metabolic acidosis,there were 80% with high AG level and metabolic acidosis and 20% with triplicate acid base imbalance.Conclusion The results indicated that newborns with asphyxia had significant even severe acid base imbalance,and mainly have displayed high AG level,with high incidence rate of metabolic acidosis.High AG metabolic acidosis had nothing with the asphyxia degree(P>0.05);the serum concentration of Cl-and HCO3- of high AG cases was obviously lower than normal AG cases or low AG cases(P<0.05).So we should determine AG level of neonatal asphyxia besides blood gas analysis so as to make more accurate diagnosis in acid base imbalance,to correct acid base and electrolyte disturbances,meantime to have to clear out airway and improve microcirculation actively. ......
