【摘要】 目的 探讨利培酮、氯氮平、氯丙嗪对首发精神分裂症患者生活质量的影响。方法 将117例首发精神分裂症患者随机分为利培酮、氯氮平、氯丙嗪组各39例,对三组经3mo治疗、潘氏量表评分减分率≥50%的患者进行1a随访，并采用生活质量综合评定问卷评定患者的生活质量。结果 利培酮对患者的躯体功能、心理功能、社会功能及生活质量改善明显，氯氮平次之，氯丙嗪较差。相关分析显示：服药剂量越大，N分越高，可能会影响患者的生活质量。多元回归分析表明，出院后服药剂量、种类、阴性症状是影响患者生活质量的重要因素。结论 利培酮不仅能改善患者的生物学症状，而且对患者的心理功能及社会适应能力有同步的调整作用。【关键词】 生活质量；精神分裂症；利培酮；氯氮平；氯丙嗪
Comparative study on the life quality of firstepisode schizophrenics treated with risperidone, clozepine and chlorpromazine

【Abstract】 Objective To compare the life quality of three groups of schizophrenic patients who were treated with risperidone, clozapine and chlorpromazine respectively. Methods 117 schizophrenics were randomly divided into risperidone, clozapine and chlorpromazine group(each n=39) for 3 months. After 3 months, patients with scorereducing rate ≥50% of the PANSS were followed up for 1 year and their life quality assessed with the Generic Quality of Life Inventory74(GQOLI74). Results Risperidone had obviously improved patients' somatic, mental and social function and quality of life, clozapine came next and chlorpromazine was the poorest. Correlation analysis showed that larger dosage and higher N score could affect patients' quality of life. Multiple regression revealed that the dosage after discharge, the kinds of drugs and negative symptoms were important factors influencing patients' quality of life. Conclusion The resperidone not only improve patients' biological symptoms but has synchronous regulator action on mental function and social adaptability. ......
