     [内容提要]本文阐述了难治性癫痫的病因及预告因素，提出针对真性难治的药物治疗策略。新诊断癫痫的患者应首选单药治疗；当单药或2种药治疗无效时应重新评估诊断，排除非痫性事件，权衡手术的利弊，并选择合理的多药治疗。

    [Abstract]The fundamental pathology of medically refractory epilepsy and major prognostic fctors are reviewed.This article focuses on the practical approaches to the management of refractory seizures.Monotherapy remains the treatment method of Choice for newonset epilepsy,but if seizures are coly partially controlled at maximally tolerated doses,consder rational polypharmacy.If two medications do not completely control scizures,the diagnosis should be reconsidered,surgical options,or using more than two drugs also be considered.

    难治性癫痫 (refractory epilepsy)或顽固性癫痫(intractable epilepsy)是指临床经过迁延，对抗癫痫药物(AEDs)治疗反应差的一组癫痫病人，即3种一线药物采用“最理想”的剂量单独或合并使用2年以上仍有癫痫发作[1]。国内定义为：频繁的癫痫发作至少每月4次以上，应用适当的一线ADEs正规治疗 ......
