【摘要】目的：用磁激活细胞分离术(MACS)检测胃癌患者外周血和门静脉血中癌细胞，探讨胃癌血液微转移情况。方法：术前抽取23例胃癌患者和7例对照者的外周血，术中取其门静脉血，经MACS富集、分离，用流式细胞仪检测癌细胞含量。结果：未经MACS的血标本中较少发现癌细胞，富集后的标本癌细胞阳性率在胃癌组与对照组(外周血：16/23 VS 1/7,门静脉血：19/23 VS 1/7，P<0.01)以及pTNMⅠ~Ⅱ期与Ⅲ~Ⅳ期患者(外周血：0.19% VS 0.48%,P<0.01，门静脉血0.42% VS 1.89%,P<0.001)差别均有统计学意义。胃癌组门静脉血中游离癌细胞阳性率高于外周血(1.81% VS 0.42%,P<0.01)。结论：MACS能有效地富集血中癌细胞，提高癌细胞检出率：循环血癌细胞数量与胃癌病理分期密切相关。MACS检测血中癌细胞对判断肿瘤转移及选择化疗合理途径、时机、监测疗效有指导意义。【关键词】磁激活细胞分离术·胃肿瘤·肿瘤转移·游离癌细胞
Detection of blood micrometastasis with the magneticactivated cell sorting in patients with gastric cancer

【ABSTRACT】Objective:The significance of MACS(magnetic activated cell sorting)was studied for diagnosing and treating cancer micrometastasis by enriching and quantifying free carcinoma cell in peripheral and portal circulation,in gastric cancer patients with different stages.Methods:Peripheral blood samples,preoperatively collected from 23 patients with gastric cancer and 7 benign tumor volunteers,portal blood being collected during operation,were sorted by MACS;the carcinoma cells were quantified by flow cytometry and compared between different groups.Results:Carcinoma cells could rarely be detected in the samples without MACS;in the samples undergone MACS,the concentrations of carcinoma cells were significantly higher in gastric cancer group than in control group,and lower in pTNM stage Ⅰ~Ⅱ patients than in stage Ⅲ~Ⅳ patients(16/23 VS 1/7;19/23 VS 1/7;P<0.01).The free cancer cells in portal blood were more than those in peripheral blood (1.81% VS 0.42%,P<0.01).Conclusion:MACS can effectively enrich epithelial tumor cells in peripheral and portal blood,close relationships can be found between carcinoma cell concentrations and tumor clinical stages.The technique colud become a valuable tool to guide evaluation of cancer metastasis risk and the strategy of chemical therapy. ......
