试述喹诺酮类药物研究进展及临床应用

     【摘要】 目的 了解喹诺酮类药物的现状及应用概况,以更好地促进合理用药,减少细菌耐药性的发生。方法 对喹诺酮类药物的研究发展史、研究进展、临床用药状况等相关资料进行回顾性分析。结果 (1)喹诺酮类药物是一种人工合成抗菌药物,现已发展到第四个阶段,在结构上有了很大改进,药理特性上有了很大突破,抗菌活性达到或超过β内酰胺类抗生素,甚至与“泰能”相媲美;(2)化学结构与作用、光毒性有关,可以通过改造母环8位的取代基降低光毒性;(3)细菌耐药性机制主要是染色体突变;(4)有抗生素后效应,设计临床给药方案时要适当延长给药间隔,两药联用时应减小药物的单剂量;(5)由于抑制拓扑异构酶Ⅱ和抗有丝分裂作用,有抗病毒和抗肿瘤作用趋势;(6)医院用药比例、占总费用份额增加,耐药性问题已引起广泛关注。结论 喹诺酮类药物仍是一类广谱、高效、低毒的抗菌药物,应进一步有计划地研究与开发,并正确选择与合理应用。

    【Abstract】 Objective Understanding the medicine of quinolones present condition and apply of the general situation, to promote better reasonable use the medicine, reducing the germs occurrence for drug resistance. Methods Make progress to the quinolones medicine research development history, research, clinical use etc. Result (1) The medicine of quinolones is a kind of artificial to synthesize the anti-bacteria drug, having developped for the fourth stage now, having the very big improvement on the structure, having the very big breakthrough on the medicine characteristic, the activity attains or exceeds the β bactamas antibiotics, even match in excellence with TIENAM mutually; (2)the chemistry structure has something to do with function, light toxicity, can pass to reform the mother wreath is 8 of to replace to lower the light toxicity;(3)the drug resistance mechanism is a chromosome mutation primarily; (4)there is antibiotics delay effect, design clinical want the hour of medicine project the adequacy prolong to the medicine partition, two medicines unites to use should minimize the single doses of the medicine; (5)because of repressing the toposiomerose Ⅱ to have the silk abruption function with the anti-, having virus safe with anti- tumor function trend;(6)the hospital uses the medicine comparison and occupy the total expenses quota increases, bearing the medicine problem to have caused the extensive concern. Conclusion The medicine of quinolones is still an extensive and widespread, efficiently, the anti-bacteria drug of the low poison, should further plan ground research and development, and right choice and reasonable application. ......
