摘要: 目的：探讨双水平气道正压通气治疗慢性阻塞性肺病Ⅱ型呼吸衰竭的临床疗效。方法：对35例慢性阻塞性肺病Ⅱ型呼吸衰竭的患者采用双水平气道正压通气进行治疗，分析无创正压通气(NIPPV)治疗前后动脉血pH值、动脉血氧分压(PaO2)、动脉血二氧化碳(PaCO2)、心率(HR)、呼吸频率(RR)等指标的变化。结果：实施NIPPV 3 h后即能改善患者的动脉血pH值、PaO2、PaCO2,减慢HR、RR；治疗72 h后与通气前比较差异有统计学意义(P均<0.01)。结论：使用NIPPV治疗慢性阻塞性肺病Ⅱ型呼吸衰竭疗效确切，且使用时间长效果更好。

    BiPAP therapy for type Ⅱ respiratory failure in chronic obstructive pulmonary disease

Method: Artery blood gas analysis were performed for 35 COPD patients under BiPAP therapy before and after ventilation respectively. Results: Succeed rate of NIPPV was 91.43%. Invasive ventilation through endotracheal intubation was given to three patient who was failued in NIPPV therapy. pH, PaO2, PaCO2 were improved prominently after 3 hours ventilation therapy. Conclusion: NIPPV therapy has reliable curative effect on the patients of type Ⅱ respiratory failure in chronic obstructive pulmonary disease (COPD). ......
