奥扎格雷钠治疗慢性肺心病急性发作 疗效观察

【摘 要】 目的 观察奥扎格雷钠治疗慢性肺心病急性发作期的临床疗效。方法 65例慢性肺心病急性发作期患者随机分为治疗组33例和对照组32例。对照组接受常规治疗，治疗组在接受常规治疗的同时给予奥扎格雷钠静脉点滴。治疗前和疗程结束后分别观察肺动脉平均压(MPAP)、体动脉平均压(MBAP)、动脉血气指标(PaO2、PaCO2)、血浆血栓素A2(TXA2)代谢产物TXB2和前列环素(PGI2)代谢产物6KPGF1α等指标，并进行疗效比较。结果 治疗组与对照组总有效率分别为87.9%和65.6%，2组比较差异有显著意义(P<0.05)。2组均可明显改善肺心病患者的临床症状；提高PaO2、降低PaCO2；降低MPAP，而对MBAP无明显影响；减少体内TXA2的生成，改善TXA2/PGI2的比值，与治疗前比较差异均有显著意义(P<0.05或P<0.01)，但2组间比较，差异无显著意义(P>0.05)。结论 奥扎格雷钠对慢性肺心病急性发作期有较好的治疗作用。【关键词】 肺心病；奥扎格雷钠；血栓素A2

【Abstract】 Objective To observe the clinical efficacy of ozagrel in the treatment of acute onset of chronic cor pulmonale.Methods 65 patients with acute onset of chronic cor pulmonale were randomly allocated into two groups: 33 patients in treatment group and 32 patients in control group.The patients in control group received routine treatment, while the patients in treatment group received both routine therapy and ozagrel treatment.The clinical symptoms,blood gas parameters (PaO2,PaCO2),mean pulmonary arterial pressure (MPAP),mean body artery pressure (MBAP),plasma thromboxane B2 (metabolic products of thromboxane A2),and 6KPGF1α (metabolic products of PGI2) were detected before and after treatment.Results The total effective rate in treatment group was 87.9%,which was 65.6% in control group,there was a significant difference between the two groups (P<0.05).In both groups the clinical symptoms were relieved,PaO2 increased and PaCO2 decreased,MPAP was reduced,however,which didn't affect MBAP.The synthesis of TXA2 was reduced and the ratio of TXA2/PGI2 was improved.There were significant differences in these parameters before treatment and after treatment (P<0.05 or P<0.01),while there were no significant differences in these parameters between two groups (P>0.05).Conclusion Ozagrel has good therapeutic effect on acute onset of chronic cor pulmonale. ......
