股骨转子间不稳定骨折内固定疗效分析

摘 要：目的 了解股骨转子间不稳定性骨折内固定术式与疗效情况。方法 将64例股骨转子间不稳定性骨折随机分为A、B两组。A组用髓内固定针Gamma 钉，B组用滑动加压螺钉加侧方钢板Richards钉。比较两组切口长度、手术时间、术中出血量、输血量、术后引流量、复位以及并发症的发生率等。结果 Gamma钉组切口长度、手术时间、手术中出血量、输血量、术后引流量和并发髋内翻率均较 Richards组低(P<0.05或0.01)，复位优良率较Richards组高(P>0.05)。结论 Gamma钉用于股骨转子间不稳定骨折内固定效果较Richards钉优。关 键 词：骨折；股骨转子；骨折内固定术
The clinical observation of internal fixation on unstable intertrochanteric fracture

Abstract:Objective To understand the effect internal fixation on unstable intertrochanteric fracture. Methods 64 cases with unstable intertrochanteric fracture were randomly divided into two groups; A group was treated with the Gamma nail intramedullary fixation device and B group, with the Richads sliding hip screw and plate device. We compared two groups with the incision length, the operative time, the blood loss during operation, the blood transfusion, the volum of postoperative drainage, the rate of reduction and complication. Results The compared of the incision length, the operative time, the blood loss during operation, the blood transfusion, the volum of postoperative drainage and the rate of complication of hip inversion were significantly loss for A group than for B group(P<0.05 or P<0.01),while the rate of excellent or good reduction in A group was hig her than that in B group(P>0.05). Conclusion Gamma nail is an effective way to treat unstable intertrochanteric fracture, which is better than Richads nail. ......
