     【摘要】目的分析胰岛素敏感性和代谢综合征危险因素之间的关系。方法使用OGTT检查按照1999年WHO糖尿病诊断标准将155名健康体检者分为正常糖耐量组(NGT)、糖耐量受损组(IGR)和2型糖尿病组(2DM)，测定身高、体重、腰围、血压、血糖、血脂、胰岛素和炎症因子，计算体质指数(BMI)、腰臀比(WHR)和胰岛素敏感指数(ISI)。IGR 和2DM组接受3个月罗格列酮治疗。结果代谢综合征的危险因素伴随着ISI的下降同时出现，ISI和BMI、血压、甘油三酯(TG)、低密度脂蛋白胆固醇(LDLC)等呈负相关，和高密度脂蛋白胆固醇(HDLC)、脂联素(adiponectin)呈正相关。IGR和2DM人群在使用罗格列酮3个月后各种炎症指标如超敏C反应蛋白(hsCRP)、肿瘤坏死因子α(TNFα)较治疗前有明显的下降，脂联素有明显的增加。胰岛素敏感指数ISI也同时有明显的改善。结论胰岛素敏感指数ISI随着NGT向糖调节受损到2型糖尿病的发展而逐渐下降，而一些代谢综合征的危险因素也伴随着同步出现，对于代谢综合征及其并发症的预防应从早期开始。罗格列酮可以在临床中成为预防和治疗代谢综合征改善胰岛素抵抗的选择之一。

    【Abstract】 Objective To investigate the relation between insulin sensitivity and the risks of metabolic syndrome.Methods 155 healthy subjects were examined by OGTT. According to diagnostic criteria of diabetes made by WHO in 1999, the subjects were divided into three groups: 2DM group, IGR group and NGT group. Height, weight, waist, blood pressure, plasma glucose, lipid, insulin and marker of inflammation (hsCRP,TNFα and adiponectin) were measured in all subjects and ISI were calculated as well. Groups of 2DM, IGR were recepted rosiglitazone 4 mg/day for three month. After treatment serum glucose, marker of inflammation and ISI were measured. Results BMI,TG,LDLC,blood pressure,hsCRP,TNFα in IGR and 2DM groups were higher than those in NGT group.ISI in IGR and 2D M groups were lower than those in NGT group.ISI was negatively related to BMI,TG,LDLC,blood pressure. After taking rosiglitazone 4 mg/day three month,hsCRP,TNFα,plasma glucose were remarkably lower, ISI was higher than before in IGR and 2DM groups.Conclusion With the development of NGT to IGR and 2DM, ISI reduced gradually and the risk factors of metabolic syndrome also floating out of the sights.To prevent and treat Metabolic Syndrome should be started early and rosiglitazone could be one of the choice to prevent and treat metabolic syndrome. ......
