体外循环下冠状动脉搭桥手术18例临床分析 (pdf)[摘要] 目的 总结体外循环下冠状动脉搭桥手术(CABG)的早期疗效和经验。方法 体外循环下行CABG 18例，其中稳定型心绞痛4例，不稳定型心绞痛10例，心肌梗死后心绞痛4例。冠脉造影提示：左主干病变10例，二支病变4例，三支病变14例。左心室射血分数(LVEF)35%～56%，平均49.5%。采用低温冷血间断灌注保护心肌，体外循环时间平均(114±29)min，主动脉阻断时间(74±22)min。术中搭桥2～4支，平均(3.1±0.7)支。合并瓣膜置换2例，合并室壁瘤4例。结果 全组无手术死亡病例，均顺利恢复，出院时心绞痛完全缓解，心功能Ⅰ～Ⅱ级。结论 体外循环下CABG早期效果较好，再血管化充分，能解除心绞痛，改善心功能，提高患者生活质量。

[Abstract] Objective To summarize the early results and experiences of coronary artery bypass grafting in coronary artery disease.Methods 18 cases were performed conventional coronary artery bypass grafting operations,including 10 cases of unstable angina,4 cases of stable angina,4 cases of angina after myocardial infarction.The coronary arteriography indicated that 10 cases complained with left main stem disease,4 cases of twovessel disease and 14 cases of threevessel disease.LVEF was between 35% and 56%,49.5% in average,oxygenated cold cardiloglegia was used for myocardic protection in all patients.the cardiopulmonary bypass lasts(114±29)min.The aortic clamped time lasts (74±22)min.2～4 vessel,(3.1±0.7) vessel were bypass grafting in these operations.Among them,there were aneurysm on left ventricular wall in 4 cases and valve disease in 2 cases.Results After the operation,all the patients had no deaths,no angina pectoris,cardiac function had improved to class Ⅰ.Conclusion CABG is an efficient way to treat the coronary artery disease.It improves cardiac function,cures agina pectoris,and makes patients live well. ......
