[摘要] 目的 探讨大剂量丙种球蛋白治疗中、重度病毒性心肌炎的效果。方法 将48例中、重度病毒性心肌炎病儿随机分为两组，对照组常规治疗，治疗组在常规治疗的基础上，给予大剂量丙种球蛋白治疗，观察用药后两组病儿临床疗效、临床症状恢复时间、心电图、心肌酶谱、左心室射血分数(LVEF)、左心室短轴缩短率(LVSF)、E峰、A峰、E/A比值。结果 治疗组疗效优于对照组(u=2.26、6.56,P<0.05)；治疗组临床症状恢复时间较对照组短；治疗组心电图、心肌酶谱阳性例数与对照组比较有明显差异(χ2=4.61、6.05，P<0.05)；治疗组LVEF、LVSF与对照组比较差异有显著性(t=2.87、3.06,P<0.01)；治疗组E峰、A峰、E/A比值与对照组比较无显著性差异(t=0.38～0.98,P>0.05)。结论 大剂量丙种球蛋白冲击治疗中、重度病毒性心肌炎疗效确切。[关键词] 丙种球蛋白类 ；心肌炎 ；治疗结果
EFFECT OF HIGHDOSE IMMUNOGLOBULIN ON MODERATE TO SEVERE VIRAL CARDITISQIU LI， FENG BING， JIANG ZHAOLUN， et al(Department of Pediatrics， Peoples Central Hospital of Tengzhou, Tengzhou 277500, China) [ABSTRACT]ObjectiveTo study the effect of highdose immunoglobulin on moderate to severe viral carditis.Methods Fortyeight patients were randomly divided into two groups. The control group was given conventional therapy. The research group was given highdose immunoglobulin in addition to conventional treatment. The time of recovery, electrocardiography, cardiac enzymes, left ventricle ejection fraction (LVEF), left ventricle shorting fraction (LVSF), peak A, peak E, and ration of E/A were measured after treatment.ResultsThe effect in the research group was better than that in the control group (u=2.26,6.56; P<0.05). The differences were significant between the two groups in terms of clinical symptoms, electrocardiography, and cardiac enzymes (χ2 =4.61,6.05; P<0.05). The LVEF and LVSF in the research group changed significantly (t=2.87,3.06; P<0.01).
