瑞芬太尼氯胺酮静脉全麻在小儿包皮环切术中的应用

【摘要】 目的 探讨瑞芬太尼氯胺酮静脉全麻用于小儿包皮环切术的可行性。方法 择期包皮环切术患儿40例,ASA Ⅰ级。随机分为A、B 2组，各20例。A组瑞芬太尼6 μg·kg-1·h-1持续静脉滴注+氯胺酮1.5 mg/kg静脉注射后手术;B组咪唑安定0.1 mg/kg+氯胺酮2 mg/kg静脉注射后手术，术中依需要静脉注射氯胺酮加深麻醉。比较2组的麻醉效果、呼吸与循环的变化、氯胺酮用量及苏醒时间。结果 2组均获良好麻醉效果；A组血压和心率稳定，B组血压和心率升高(与A组比较,P<0.01);从呼吸次数看A组较B组易引起呼吸抑制(P<0.01)，A组氯胺酮用量及苏醒时间均少于B组(P<0.01)。结论 瑞芬太尼氯胺酮静脉全麻疗效确切,苏醒快而完全，可用于小儿包皮环切术。【关键词】 瑞芬太尼；氯胺酮；小儿；包皮环切术

【Abstract】 Objective To investigate the feasibility of remifentanilketamine intravenous anesthesia used for circumcision in children.Methods 40 children,underwent circumcision selectively,ASA Ⅰ,were divided into group A and group B randomly. Remifentanil 6 μg·kg-1·h-1 iv continuously + ketamine 1.5 mg/kg iv was used in group A, while midazolam 0.1 mg/kg and ketamine 2 mg/kg iv was used in group B, both groups were followed by operation. Ketamine was administrated intravenously to deepen anesthesia during operation.Results The anesthesic effects were excellent in both groups. Blood pressure and heart rate in group A were kept stable, while those in group B were significantly increased (P<0.01).The respiratory depressionwas more serious in group A than that in group B(P<0.01).Ketamine dose consumption and awaking time in group A were significantly less than those in group B (P<0.01).Conclusions With reliable effect, quick and complete consciousness, remifentanil-ketamine intravenous anesthesia can be used for circumcision in children. ......
