摘要:目的: 探讨呼吸机相关性肺炎(ventilator-associate pneumonia VAP)病原菌及药敏情况。方法: 总结分析我院2002年1月至2006年5月住院病人62例VAP病原菌及耐药情况。结果:VAP的主要致病菌革兰阳性菌占14.2 %,革兰阴性菌占78.8%。分别列前3 位的是肺炎克雷伯杆菌47%、鲍曼不动杆菌17%、铜绿假单胞杆菌11%。肺炎克雷伯杆菌对亚胺培南,哇诺酮类敏感率高,对添加β-内酰胺酶抑制剂的抗生素(头孢哌酮-舒巴坦和哌拉西林-三唑巴坦)敏感率在50~60% ,对氨基糖甙类及青霉素类、头孢2 、3代敏感率低。结论:VAP的主要致病菌为肺炎克雷伯杆菌,药敏试验可作为临床经验用药参考。关键词: 呼吸机相关性肺炎; 病原体; 耐药性
Pathogenic Bacterium and Susceptibility Test Analysis of Ventilator Associate Pneumonia

Abstract:Objective:We investigated the pathogenic bacterium susceptibility test of ventilator –associate pneumonia.(VAP). Method: We reviewed the pathogenic bacterium and susceptibility test of the 62 VAP cases in our hospital from January 2002 to May 2006.Result: The result displayed the composition of the main pathogenic bacterium of VAP: Gram-positive bacteria hold 14.2%, while Gram-negative bacteria hold 78.8%, the first three pathogenic bacterium is Klebsiella pneumoniae(47%); baumanii(17%); Pseudomonas aeruginosa(11%). And we also investigated that Klebsiella pneumoniae has high sensitive to the imipenem and fluroquinolone,especially the antibiotics which contain the extended-spectrum β-lactamase inhibitor such as cefoperazone-sulbactam and piperacillin-Tazobactam have the sensitivity to 50~60%, while to the aminoglycosides; the penicillins and the cepbalosporin P2 ......
