冶金工业50岁以上人群冠心病、脑卒中危险因素调查及干预研究

     【摘要】 目的 在广东韶钢(CSG)50岁以上人群开展冠心病、脑卒中多重危险因素调查,并进行干预管理及危险因素控制,以减少冠心病、脑卒中的发病率。方法 用随机方法挑选病例200例,分为干预组(A组)、对照组(B组),干预组按照北京阜外心血管病医院《心血管病多重危险因素干预方案》给予干预,主要为非药物干预,对照组不干预,时间2年,记录两组急性心血管病事件发病率,危险因素主要记录血压、血脂、血糖、肥胖、吸烟、运动等情况的变化。结果 (1)两组急性心脑血管病事件发病率分别为冠心病4%、15%,脑卒中7%、19%(P<0.001);(2)A组危险因素控制:血压收缩压下降20%、舒张压下降15%,总胆固醇下降30%、甘油三酯下降13%,空腹血糖(FBG)下降10%,体重指数下降10%,吸烟减少60%,与干预前对比差异有显著性(P<0.001),B组则无差异。 结论 在冶金工业50岁以上人群开展冠心病、脑卒中多重危险因素调查并进行干预,可以减少冠心病、脑卒中的发病率。

    【Abstract】 Objective Opening an exhibition research variety dangerous factor, with carry on an intervention management and control dangerous factors of coronary heart disease and stroke above the 50 ages in CSG, to reduce an outbreak rate. Methods Choosing 200 cases with the random method, is divided into an intervention set (A set), matched control (B set), A set according to the cardiovascular disease Fuwai Hospital and Cardiovascular Institute Beijing《The intervention project of varity dangerous factors of cardiovasular disease》, the main right and wrong medicine interferes, B set doesn't interfere,time is two years, record two sets inpatient and cardiovascular disease affair outbreak rate, the dangerous factors mainly records the variety of circumstance of blood pressure, blood fat, blood glucose, fat, smoking, exercise etc. Results (1) The outbreak rate of two sets respectively is coronary heart disease 4%, 15%, stroke inducement 7%,19%, P<0.001. (2) Control a dangerous factors of A set: the systolic BP to descend 20%, the diastolic pressure 15%, total cholesterol 30%, triglyceride 13%, FBG 10%, weight index 10%, smoking 60%, to have obrious difference, P<0.001, B set then make no difference. Conclusion The dangerous factor investigate and interventional research of coronary heart disease and stroke above the crowd of 50 ages in the metallurgy industry, can reduce an incidence rate. ......
