    经自然腔道内镜手术在心胸外科中的研究及应用进展

    作者简介：通讯朱成楚，主任医师、教授，温州医科大学硕士生导师，享受国务院特殊津贴。现任浙江省台州医院心胸外科教授、主任医师，浙江省台州恩泽医疗中心(集团)党委书记。担任浙江省医学会胸心外科学分会副主任委员兼胸腔镜学组组长，为浙江省医学会微创外科学分会副主任委员，中华医学会胸心血管外科学分会胸腔镜外科学组委员，《中华外科杂志》通讯编委、《中华胸心血管外科杂志》等多个杂志编委。近年主持浙江省重大科技专项和优先主题项目、浙江省公益性技术应用研究计划、浙江省卫生厅重点项目、浙江省2011年度第一批省级重点科技创新团队项目等多项课题项目。曾获得全国卫生系统先进工作者，浙江省科技进步二、三等奖，浙江省有突出贡献中青年专家等荣誉。

    【摘要】经自然腔道内镜手术(NOTES) 是指利用人体的自然腔道(如胃、阴道、膀胱等)，不经皮肤切口，将内镜穿破腔道管壁进入体腔进行内镜下手术的外科学技术，多用于腹部外科、泌尿外科等。胸部的解剖特点使得在该部位实施NOTES的手术风险较高。为提高临床工作者对NOTES的认识，该文就NOTES在心胸外科中的应用条件、腔道入路的选择及方式、闭合方法、手术风险、研究进展、存在问题及应用前景等进行了介绍。

    Abstract【】Natural orifice translumenal endoscopic surgery (NOTES) is a technique that allows insertion of an endoscope into the peritoneal cavity through natural orifices (such as, stomach, vagina and urinary bladder, etc.) rather than skin incisions, which has been widely applied in the abdominal and urological surgeries, etc. Due to the characteristics of thoracic dissection, it is of relatively high risk to perform thoracic NOTES. To enhance the understanding of NOTES in clinician, the application condition, selection and pattern of orifice approaches, closure methods, surgical risk, research development, existing problems and prospect of application were summarized in this study. ......
