【摘要】目的:探讨肱骨近端骨折切开复位锁定钢板内固定的治疗疗效。方法:对2006年-2009年12月通过手术治疗肱骨近端骨折60例进行回顾性分析。按Neer分型二部分10例、三部份30例四部份20例按AO分型A型:10例 B型:30 C型20例。结果:有60例患者全部随诊,对NeerIII分型三、四部份骨折 AO:B2.3型C型:比较:用张力带克氏针固定5例,切开复位解剖钢板固定20例 锁钉钢板25例, 二型用锁定钢板例5例普通钢板5例。按Neer三、四部份骨折 AO分型:B2.3型 C型:用锁钉钢板内固定肩关节功能恢复满意无肱骨头坏死。用张力带克氏针解剖钢板固定肩关节功能较健侧差30/% ,关节屈伸与疼痛。Neer二型解剖钢板和锁钉钢板差别不明显。结论:切开锁定钢板内固定治疗股骨近端骨折疗效好并发症少尤其是Neer三、四部分骨折AO B、C骨折有明显的优势术后配合合理的康复锻炼肩关节能达到最大程度的恢复。
