【摘要】孔桥社区卫生服务中心通过制订总体规划,搭建工作平台,争取政府支持,鼓励社区参与;开展社区诊断,多种渠道发现慢病患者,采取多种健康教育,促进了防治工作的顺利开展,管理取得初步成效。慢性病报告率达到85%以上,建档率为100%,随访管理率达到80%以上,健康教育覆盖率达到90%以上。
