高尿酸血症与2型糖尿病肾病关系的探讨

【摘要】 目的 探讨高尿酸血症与2型糖尿病肾病(T2DM)的关系。方法 将100例T2DM患者分为DN组(55例)与DN-HUA组(45例)。另择50例健康人作为正常对照(NC)组。同时检测三组的尿微量白蛋白排泄率(UAER)、糖化血红蛋白(HbA1c)、肌酐(Cr)、尿素氮(BUN)、血尿酸(UA),进行相关的统计学分析。结果 三组HbA1c、Cr、BUN、UAER、UA值比较差异有统计学意义(P<0.01)。DN组和DN-HUA组的Cr、BUN、UAER、UA和HbA1c值分别高于NC组(P<0.01),DN-HUA组的Cr、BUN、UAER、UA值均高于DN组(P<0.01),DN-HUA组和DN组的HbA1c水平差异无统计学意义(P>0.05)。结论 高尿酸血症是糖尿病肾病的一个重要危险因素。

Relationship between hyperuricemia and Type 2 diabetic nephropathy

【Abstract】 Objective To explore the relationship between hyperhomocysteinemia and diabetic nephropathy(DN).Methods 100 patients with T2DM were divided into Group DN (55cases) and Group DN-HUA(45cases) and 50 healthy people were chosen as Group NC. The serum HbA1c,UA,BUN,Cr,and UAER in the 3 groups were simultaneously detected and relevant statistic analysis was made. Results The values of the HbA1c,Cr,BUN,UAER and UA in the 3 groups showed significant difference(P<0.01). The values of Cr,BUN,UAER,UA and HbA1c in Group DN and Group DN-HUA were averagely higher than those in Group NC(P<0.01). The values of Cr,BUN,UAER and UA in Group DN-HUA were averagely higher than those in Group DN(P<0.01). The HbA1c levels of Group DN-HUA and Group DN had no significant difference(P>0.05).Conclusion Hyperuricemia is an independent risk factor in DN.

糖尿病肾病(DN)是最常见的糖尿病微血管并发症之一,后期可导致慢性肾功能衰竭。近年高尿酸血症的发病率升高,嘌呤代谢紊乱是其基本原因。高尿酸已成为肾功能异常的独立危险因素[1,2]。本研究旨在探讨血尿酸水平变化在DN发病机制中的作用,现报道如下。
资料与方法1.一般资料 选取收治的无血缘关系的T2DM患者100例,均符合1999年世界卫生组织(WHO)的糖尿病诊断标准。其中DN无血尿酸升高患者(DN组)55例,男30例,女25例,年龄(59.4±14.1)岁。诊断标准:尿微量白蛋白排泄率(UAER)>30 mg/24 h或血肌酐(Cr)>133 μmol/L,尿素氮(BUN)>7 ......
