    【摘要】目的：探讨治疗肝内胆管结石的方法及临床疗效。方法：回顾性分析168例复杂性肝内胆管结石病人行外科治疗的资料，对比不同治疗方法的临床效果，总结各种方法在临床治疗中的作用和适应证。肝切除术式采用左外叶切除50例(32.26%)，左半肝切除39例(25.16%)，肝方叶切除12例(7.74%)，右前叶切除5例(3.22%)，右后叶切除20例(12.90%),右半肝切除5例(3.2%),双侧多区段肝切除36例(23.22%)；其中附加胆肠内引流术78例(58.32%)，经肝实质切开取石4例。结果：根据手术探查的胆管狭窄部位及范围和结石状况，采用不同的手术方式。结论：正确选择肝内胆管结石手术方式是减少术后残留结石的关键，而规则性肝叶/段切除是治疗原发性肝胆管结石的有效手段。
