【摘 要】目的：探讨基于家庭医生团队管理的2型糖尿病家庭支持路径与效果评价方法。方法：选取150名糖尿病患者及其家庭，由家庭医生小组通过健康教育、行为指导、家庭协同等强化家庭支持12个月，分析评估管理前后患者糖尿病相关认知、遵医依从性、理化指标情况等。 结果： 基于家庭支持下的社区糖尿病综合管理后，居民糖尿病认知水平、依从性、部分理化指标及家庭支持行为明显优于管理前(p<0.05，p<0.01)。结论： 家庭支持协同管理模式在社区糖尿病健康管理中效果明显，适宜推广，但家庭医生角色扮演，家庭干预管理路径尚需进一步优化与思考。

【Abstract】Objective： To explore the family support pathway and effect evaluation method of type 2 diabetes mellitus based on family doctor team management.. Method： 150 diabetic patients and their families were selected. Family doctor group strengthened family support through health education， behavior guidance， family coordination for 12 months. The related knowledge， compliance with medical treatment， physical and chemical indicators of diabetes before and after management were analyzed and evaluated. Result： After comprehensive management of community diabetes based on family support， residents'diabetes cognitive level， compliance， some physical and chemical indicators and family support behavior were significantly better than those before management.(p<0.05，p<0.01) Conclusion： Family support collaborative management model has obvious effect in community diabetes health management and is suitable for promotion. However， family doctor's role-playing and family intervention management path still need further optimization and reflection.

随着现代医疗模式的转变，糖尿病的控制不是传统意义上的治疗而是系统的管理[1]。大量循证医学证实，包括生活方式干预、健康教育、指标监测、药物治疗等综合管理措施是控制血糖，降低并发症及心脑血管事件的主要措施。梁瑶梦等[2]研究表明，家庭内、外支持与糖尿病患者不遵医行为呈正相关。家庭支持、社区协同、专业引导、自我管理可能是糖尿病从“疾病管理”向“健康管理”转变的关键一步。
为进一步探索基于家庭支持的糖尿病综合干预路径，引导社区、家庭、社会共同关注糖尿病，形成“自我管理-家庭支持-社区协同”的糖尿病管理新模式。我们以家庭医生小组为单元，在前期现况调查的基础上，开展了本项目的探索研究。, 百拇医药(姚裕忠 吴文怡 邵慧)
