不同术式治疗慢性闭角型青光眼合并白内障的疗效比较(1)

     【摘 要】目的：研究不同术式治疗慢性闭角型青光眼合并白内障的临床效果。方法：随机抽取92例慢性闭角型青光眼合并白内障患者，采用随机综合序贯法分为参照组和治疗组各46例。参照组采用小切口囊外摘除联合人工晶体植入术及小梁切除术治疗，治疗组采用超声乳化联合人工晶植入及小梁切开术或房角分离术。手术前对患者视力及眼内压进行记录，术后5个月后再次治疗组其视力及眼内压，进行对比治疗组；同时记录患者术后并发症发生率。结果：(1)记录结果显示，治疗组与参照组治疗前视力及眼内压指标对比无差异，无统计学意义(P>0.05)。治疗后，治疗组视力明显提升，眼内压明显降低，有统计学意义(P<0.05)。(2)治疗组并发症发生率(2.17%)，参照组并发症发生率(17.39%)，有统计学意义(P<0.05)。结论：不同术式治疗慢性闭角型青光眼合并白内障具有不同疗效，采用超声乳化摘除术联合人工晶植入术及青光眼小梁切开术治疗效果更为显著，可准确减少并发症发生，提高治疗效果。

    Comparison of therapeutic effects of different surgical procedures on chronic angle-closure glaucoma with cataract

    Abstract：Objective To study the clinical effect of different surgical methods in the treatment of chronic angle closure glaucoma with cataract.Methods： 92 patients with chronic angle-closure glaucoma complicated with cataract were randomly divided into two groups： the control group (n = 46) and the treatment group (n = 46).The control group was treated by small incision extracapsular extraction combined with intraocular lens implantation and trabeculectomy，while the treatment group was treated with phacoemulsification combined with artificial crystal implantation and trabeculectomy or angle separation.The visual acuity and intraocular pressure (IOP) were recorded before operation，and the visual acuity and intraocular pressure (IOP) were recorded 5 months after operation in the retreated group，and the postoperative complications were recorded at the same time.Rate Results： (1) the results showed that there was no significant difference in visual acuity and intraocular pressure between the treatment group and the control group (P > 0.05).After treatment，the visual acuity and intraocular pressure in the treatment group were significantly improved (P<0.05).The incidence of complications in the treatment group (2.17%) was significantly higher than that in the control group (17.39%) (P<0.05).Conclusion： different operative methods have different therapeutic effects on chronic angle-closure glaucoma complicated with cataract.Phacoemulsification combined with artificial crystal implantation and trabeculectomy of glaucoma are used to treat them.The therapeutic effect is more remarkable，can accurately reduce the occurrence of complications and improve the therapeutic effect.
