甲基强的松龙治疗重症哮喘疗效观察

【摘要】目的 比较全身用甲基强的松龙(甲强龙)与地塞米松治疗重症哮喘 的疗效比较。方法 60例重症哮喘患者分为两组,治疗组32例,对照组28例; 两组均在常规治疗相同基础上,治疗组(甲强龙组) 静脉输注甲基强的松龙40～80mg/次,每 日2次。对照组(地塞米松组)静脉输注地塞米松10mg/次,每日2次。结果 两组患者治疗72小 时后临床疗效、测定动脉血气分析情况,进行相关统计学分析,两组比较差异有统计学意义(P <0.01)。结论 在治疗支气管哮喘重度发作过程中甲基强的松龙早期缓解 哮喘的疗效更佳。

【Abstract】Objective (Armor hadrosaurus) treats the critically i ll asthma's curative effect compared with the whole body with the methyl strongloose dragon with the dexamethasone to compare.Method 60 example critically ill asthma patient divides into two groups, the treatment group 32 ex amples, the control group 28 examples; Two groups in conventional treatment samefoundation, treatment group (armor hadrosaurus group) vein infusion methyl stro ng loose dragon 40～80mg/time, daily 2 times. Control group (dexamethasone group ) vein infusion dexamethasone 10mg/time, daily 2 times.Result Twogroup of patients treat 72 hours later the clinical curative effect, the determ ination artery courage vigor analysis situation, carries on the related statisti cs analysis, two group of comparison differences have statistics significance (P <0.01).Conclusion In the treatment of bronchial asthma and severeattack during methylprednisolone in the early relief asthma better.

重症哮喘是一种致命的疾病,糖皮质激素为哮喘病人的主要治疗手段。为了探讨甲基强 的松龙在重症哮喘治疗中的临床效果,我们于2007年9月到2009年9月用甲基强的松龙冲击治 疗32例重症哮喘患者,并与地塞米松相比较,取得很好的效果,现报告如下。
1 资料与方法
1.1 一般资料 选择我院2007年9月到2009年9月的60例住院患者,符合中华医学会呼吸病 分会制订的支气管哮喘指南诊断标准[1],按就诊时间分为两组,治疗组32例:男 性16例,女性16例,年龄20-61岁;对照组28例,男性15例,女性13例,年龄17-56岁,两组 患者性别、年龄、病程各项参数差异均无统计学意义(p>0.05),具有可比性。
1.2 治疗方法 所有患者入院后均给予常规治疗,包括吸氧、静脉补液、合并下呼吸道感 染时选用病原体敏感的抗生素,治疗组给予甲基强地松龙40～80mg加NS 100ml,1h内静滴完 ,每日2次静滴,共3天,对照组给予地塞米松10mg加5%GS250ml,每日2次静滴,共 3天。
1.3 观察方法 根据支气管哮喘防治指南中患者症状的严重程度分级进行日间和夜间评 分:(1)日间症状评分:0=白天无症状,1=每日有1次短暂症状,不影响生活、睡眠;2 =每日有2次以上短暂症状,可响生活和睡眠,3=白天大部分时间有症状,影响患者的生活 和睡眠 ......
