【摘要】目的 探讨轻量型可吸收疝修补装置在疝环充填式无张力疝修补术后的效果。方法 对比分析大连大学附属新华医院自2009年6月至2010年6月205例患者实施疝环充填式无张力疝修补术的临床资料。结果 所有病例均行疝环充填式无张力疝修补术，共分两组：超普网塞(UPP)组(108例)和普理灵网塞(3DP)组(97例)。本资料显示在术后早期阴囊肿胀和术后复发率方面比较，两组之间(P>0.05)无显著差异性；而在局部异物感和慢性疼痛方面比较，UPP组的发生率明显低于3DP组(P<0.05)，具有显著差异性。结论 超普网塞作为第三代轻量型部分可吸收复合网片，应用于疝环充填式无张力疝修补术的疗效确切，术后局部异物感和慢性疼痛等并发症较少，适宜广泛推广和应用。

Analysis for lightweight absorbable mesh in the plug-mesh tension-free hernioplasty

【Abstract】Objective To explore the effect of lightweight absorbable mesh in the plug-mesh tension-free hernioplasty. Methods From June 2009 to June 2010, the clinical data of 205 patients offered plug-mesh tension-free hernioplasty were analyzed comparatively. Results All patients were offered plug-mesh tension-free hernioplasty. They were divided into two groups: the Ultrapro Plug(UPP)group(n=108)andthe Prolene 3D patch(3DP)group(n=97). The hematoma of scrotum and the recurrent rate are not significant difference(P>0.05)among the two groups. But the foreign body sensation and the chronic pain are significant difference(P<0.05)among the two groups. Conclusions The Ultrapro Plug is the 3rd generation lightweight absorbable mesh. The postoperative effects are definite offered plug-mesh tension-free hernioplasty. The complication of postoperative foreign body sensation and the chronic pain is less. It can be used extensively.

自从1989年美国医师Lichtenstein提出了无张力疝修补手术的新概念以后，在疝修补手术中使用网片修补已经越来越广泛。我国较大规模的使用网片修补技术也已经历时10余年，其优点已被广大外科医师肯定，所应用的网片及疝修补装置的种类也很众多。大连大学附属新华医院于2009年6月以来采用轻量型(部分可吸收)疝修补装置行无张力疝修补术治疗腹股沟疝108例，取得了满意效果，现总结分析如下：
1 临床资料和方法
1.1 一般资料 收集大连大学附属新华医院自2009年6月至2010年6月实施在疝环充填式无张力疝修补术108例患者的临床资料，根据所采用美国爱惜康公司生产的不同网片的分为2组：(1)普理灵网塞(Prolene 3D patch ，3DP)组97例；(2)超普网塞(Ultrapro Plug，UPP)组108例。3DP组男87例，女10例；年龄28～82岁，平均59岁；发病时间5个月～11年。UPP组男98例，女10例；年龄29～89岁，平均61岁；发病时间8个月～10年。其他情况见表1。
疝的分型按中华外科学会疝与腹壁学组于2003年8月制定的成人腹股沟疝、股疝手术治疗方案(修订稿)中的分型法[1] ......
